Has your student met the health requirements to start school in August? D87 is hosting the Carle Mobile Clinic at BJHS on Friday, August 4th from 9am to 4pm. If your student still needs an updated physical or immunizations call 309.827.6031 to make an appointment or request additional information. Please bring an insurance or medical card as they will be needed at the time of the appointment.

We will have walk-in registration on August 3rd from 9-11am and 2-5pm. Please be sure to bring 2 forms of proof of address (utility bill, lease/rental agreement or mortgage papers), a certified copy of the birth certificate, Illinois physical, dental and vision forms along with immunizations for each child. If you have any questions you can call the office at 309-829-7034 starting August 1st. Before August 1st you can contact the District 87 Office at 309-827-6031.
